Please fill out the form below to nominate a veteran or active duty service member to receive a wish from Veteran’s Wish. Make sure to include a detailed description of the wish and why you are making the nomination.

The “Wish Nomination Form” is the first step in the nomination process. It is not a confirmation of eligibility for a Veteran’s Wish. Your information will be sent to our Wish Committee for review.

To be a recipient of a Veteran’s Wish, an individual must be an honorably discharged veteran, or currently serving as an active duty member of the Armed Services of the United States of America. Military documentation is required.
